Our primary focus
is to properly take care of your dental health needs; however, it is
also essential that we take care of the business aspects. The following
information is provided to assist this part of our service relationship.
The following
payment options are available:

INSURANCE-
The entire amount due is considered an obligation
between our office and the patient, with insurance billing provided
as a patient service. In the event that the insurance billing is not
successful, payment for services rendered will be expected. If we
can verify your insurance coverage, we will estimate the applicable
coverage and bill your insurance company appropriately. The balance
(co-payment) will be your responsibility and is expected at the time
of service.

CASH
OR CHECK- If you do not have insurance coverage, payment
in full is expected at the time of service. If full payment is made
with cash or check on the day of service, you will receive a 5%
discount. There is also an additional 5% discount for seniors.

DEBIT
CARD, MASTERCARD, VISA, DISCOVER- These are all
acceptable forms of payment on the day of service, but no discount
is given.

FINANCING-
We can assist you in securing financing for your larger treatment
needs. This may be helpful in allowing you to receive needed
treatment in a timely manner.
If you have
special financial needs, please bring these to the attention of our
front office staff.
